The WWE announced shortly before Sunday’s Elimination Chamber event that Rusev didn’t receive medical clearance to wrestle in the Intercontinental Championship match on the card.
Rusev even tweeted his thoughts out loud:

Rusev’s health became a question mark after the company revealed Wednesday that he suffered a lower-leg injury during Tuesday’s SmackDown tapings. A WWE fan with the Twitter handle @_BellaFam posted a photo of the Bulgarian Brute, and a boot on his right leg was clearly visible:

His injury is unlikely to have a major effect on Sunday’s title match. Mark Henry ended up replacing Rusev in the six-man bout, per WWE:
Where Rusev’s absence might hurt is in his feud with Dolph Ziggler and in the bigger picture his continued storyline with Lana. His breakup with Lana has been a major angle over the past few weeks, and a major injury to Rusev threatens to drain some of the heat between the two.
